We are a paperless electronic practice. 

Please find our office policies & paperwork below.

Once you have scheduled your appointment, you will receive an email with a link to complete our paperwork online. Or you can print the PDFs below and bring them with you. You may also arrive 30 minutes prior to your appointment time, and we can provide you copies to complete in our office.

Please bring your insurance card, a valid photo ID, and a list of your current medications to each appointment.
